I am able to verify insurance coverage, deductible, copayments, and obtain authorization for treatment when required.  Prior to scheduling, please email your insurance information including name of insurance company, patient name and date of birth, subscriber name and date of birth, insurance company telephone number, and insurance ID number.  Thank you.
